Self Reflection In Nursing
I cared for my patient on 11/06 on P5 room 10. I was given report from the leaving nurse about my patient shortly after I got on the floor. The nurse that I was assigned to was from the ICU and she was a floating nurse for that day. My patient was an 8 week old baby boy who came into the hospital on 11/03 for failure to thrive. During the report, the nurse showed us where the position of NG tube on the patient. After the report, the nurse and I did a whole physical assessment, and vital on the patient. First the nurse did the physical assessment while I was doing the vitals, then we switched and I did the vital and she did the physical assessment. I thought it was very nice that we did it together because it showed how well I was able to do vitals and …show more content…
I talked with the father about my patient’s condition last night. He mentioned that he was up with the patient last night while the patient’s mother went to Ronald McDonald’s house to rest. After the patient was fed, he went to sleep and his parents continued on with their television. Afterward, I went and charted in the computer. While I was charting, the nurse asked me to get the patient’s weight. I had to take all of the patient’s clothes and diaper off before I weighted the patient. The patient’s weight slightly gained since yesterday. Right before 11, the team members did their rounds. I was present while they were rounding. Their goal was for the nurse to help educate the parents to provide a well-balanced diet and educate proper use of the NG tube before discharging. An unclear date for discharge not determine yet. Throughout the day, I felt that I learned a lot. Before this clinical, I did not know what failure to thrive means, but after working with this patient, I understood the condition of his problems. I also learned how to feed a baby with a NG tube by using gravity. I felt that I am learning and growing each day with new

Miranda, you did wonderful in this situation. We have all gone through situations like this, and I think as long as we remain nurses situations like this will keep arising. Being an advocate for the patient will never be easy, but we as nurses must keep persevering just like you did. If we allow ourselves to be intimidated by doctors, then our pledge of being a patient advocate will be null. Masters (2017) states “the ability of nurses to think critically and make sound clinical judgments is essential to providing safe, competent, and quality care” (p. 216). You did exactly what was required of you as a nurse, and I believe if you had not been persistent to the point of getting a cardiologist on the case, the outcome would have been dire…

We started our day in the hospital lobby promptly at 0630, the professor started off by explaining where on the unit each of us would be, as she usually does. I was placed in the Neonatal Intensive Care Unit (NICU) along with one other student. This week was different than previously, there were more infants and nurses and also Intermediate Care Unit (IMCU) was in a different room. Because NICU was full with nurses and infants. I started off in IMCU, where I met this wonderful nurse who I connected with and stayed the whole day talking to her, when I wasn’t checking on the other student. The nurse allowed me to shadow her with the three infants she was assigned to. At each station the…

This week in clinical, I got my first patient to take care of. I was very excited, but overwhelmed. I went in the patient's room and introduced myself. The patient stated he wants to eat breakfast, so I helped him with that and watched him eat because of him having aspiration pneumonia. My patient was a 64 year old male, diagnosed with aspiration pneumonia, with past history of stroke causing him to have left sided weakness, and was legally blind in right eye. Before helping the patient with breakfast, I quickly did a full set of vital signs. After breakfast, I did am care for my patient. After that, I did a head to toe assessment following the sheet provided by the clinical instructor. Then, I reviewed my chart more carefully and looked…

I was on my third day of residential care placement; the staff had just started to take turns for their morning tea break so I took the time to catch up on my case study patient’s medical history in the nurses’ station. Within a few minutes the Manager of the rest home ran in to gather the blood pressure machine and bandages. She informed another student nurse and myself to “take these to Max’s (pseudonym) room NOW, while I call an ambulance”.…

Rotation Day 8 today I’m in processing it was a slower day than yesterday so the processer had me help with a few things. They also asked me if I could go with a phlebotomist to drop some blood to the Operation room. When we got there I noticed read lines on the floor meaning we could not pass until we had full gowns. We waited to see if there was someone we could flag since there was no one in the front office. A person came in and we told her we had a delivery she took it and we left. When we got back they had a request to draw a patient in the SSI department I didn’t know where this was so the phlebotomist asked if I wanted to go check it out so I went. I didn’t know it we had to drive since it was in another location. We went in and the patient was in the recovery room she was alert at the moment and recognized the phlebotomist right away.…

I believe I worked on your floor last semester because I recall such a change being discussed. I did not realize the hospital is interested in changing vital signs time for the entire hospital! I can see how moving the time to 0600 would delay the night PCTs from finishing their work and the day nurses from starting their shift. Collecting vitals and blood sugars while managing meal times and shift change sounds like a time-consuming task. However, I can see this as a potentially effective change since the day nurses would receive the most recent vitals and updates about their patients. I am curious about how long the hospital plans for to test the change on the unit. Perhaps over time the unit would adjust to the change? It appears fortunate…

Physicians rely on nurses to carry out orders in a timely and precise manner. Nurses are the foundation of the health care profession because they are not only there to provide relief and concern. They are also role models for good health care. Because physicians are often in a hurry they don’t have the time needed to establish a proper relationship with their patients. Therefore, nurses are used to assess patients and communicate imperative information to physicians on behalf of patients to ensure patients receive proper care. Nurses should never hide a mistake from physicians or patients.…

My first day with my preceptor this week (Wednesday October 26, 2011) was cut short due to a school obligation and personal situation. Although I only spent five hours on my unit, I did my best to make them count. I was particularly excited for my patient assignment today because I was caring for two infants which is truly my passion. My first patient was the same patient I had cared for the previous week, a now 21-day-old baby boy admitted with a positive urine culture and blood culture. I was glad to have this patient again because I was able to further build on my relationship with the patient’s Mother and reinforce the education I had provided previously. While caring for this patient I think I demonstrated the clinical competency of evaluating the effectiveness of the patient’s plan of care utilizing critical thinking and the nursing process and revising the patient’s plan of care. For example, this patient was responding well to his antibiotic treatment every six hours, as well as consistently taking in adequate amounts of formula with each feeding. While calculating the intake and output I noticed that the patient was urinating excessive amounts. After assessing the hydration status of the infant, I questioned the primary physician regarding the need for the client to be receiving maintenance fluid at a rate of 10ml/hr. The doctor agreed that this was excessive for such a small infant and requested the…

I went to listen to report with the nurse who had my patient and in report the nurse told us about the patient. He was an elderly man who just found out that he had brain cancer. She added that he was mean and she looked at me and told me that he would probably curse me out and that she did not know how well he would do having a nursing student. My teacher heard the report. Of course I really got nervous and I was almost in tears thinking about what I have gotten myself into. Before I could go meet the patient I had to go into another room and I just said a little pray and started walking towards the patient’s room. The teacher stopped me and told me if the patient really did not want a student that she would change my rooms. On the way to the room I thought about what we…

Was I really that horrible of a nurse? Did I even deserve to be a nurse? I pushed those thoughts out of my head and told myself, “Amanda, you are a great nurse.” Of course I was. Not only did I graduate top of my nursing class, I had an ER doctor who took me under his wing and showed me everything I needed to know and more. I knew more than what some RN’s knew. I’ve never had any of my patients die on me, so I must be at least a half decent nurse. I went in the nurses’ station and called the answering service once again. After getting off the phone, I began to walk out the nurses’ station to go tell Mrs. Williams’ daughter I called the doctor back and was preparing myself for another round of badgering questions when the phone…

Today I went to the patient care unit and spent most of my time conversing with the nurse. She was very knowledgeable and very passionate about her patients. She gave me so many facts and information in such a short amount of the time that I was only able to process about a quarter of what she told me. One of the first thing we did was get some water for a patient. On our way back from getting the water, the nurse notice two guest who needed help, but were not getting the proper assistance. She told me to deliver the water, while she helped the visitors. Afterwards, we met up again and it was time for her to administer some medicine to a patient. Before we went to another patient, the nurse needed to discuss with the doctor. We waited for…
